TEAM UP for Children Symposium - A half-day conference about integrated behavioral health care for children and families.

PANEL ONE

Developing Care Team Roles for Behavioral Health Clinicians, Community Health Workers & Family Partners | Codman Square Health Center

Interested in building a strong interdisciplinary team to successfully address the behavioral health needs of children and families?

Codman Square Health Center will share its experience working as a collaborative team, in boosting the capacity of pediatric primary care to address behavioral health issues. Hear directly from an integrated behavioral health clinician, family partner, and pediatric primary care provider on the development of their respective roles in supporting behavioral health care, and offer your ideas on the training and organizational aspects of building a strong team.


PANEL TWO

Supporting Early Childhood Mental Health & Enhanced Screening using the Survey of Wellbeing of Young Children (SWYC) | Lowell Community Health Center

Thinking about to how to improve the identification of early childhood developmental and social emotional concerns in your setting?

The SWYC is a comprehensive, Massachusetts-approved screening tool that assesses developmental milestones, behavioral/emotional development, and family risk factors, including postpartum depression. Lowell Community Health Center will discuss their experience implementing the SWYC with multi-cultural, multi-lingual families. We will share creening results from the TEAM UP health centers and discuss how the tool has impacted clinical practice.


PANEL THREE

Clinical Models and Reimbursement Strategies for Integrated Care | The Dimock Center

Wondering about how to optimize revenue from integrated behavioral health care in pediatrics?

The Dimock Center will share how they’ve developed their integrated care model with an eye towards long-term sustainability - including specifics on coding and billing, optimizing revenue, and managing provider productivity. We’ll discuss how the TEAM UP health centers have developed mechanisms to track revenue and productivity, and we’ll strategize on how to approach sustainability in the emerging ACO environment.

TEAM UP for Children seeks to enhance the capacity of seven local community health centers to meet children’s behavioral health needs by designing and testing a state-of-the-art model of fully integrated pediatric health care. Our aim is to promote positive child health and well-being through innovation and consistent delivery of evidence-based integrated care.
